I have a really painful ear infection on both sides of the ears. When I prostrate it puts a lot of pressure on my ears and make them hurt worse after. Can I do prayers by tilting my head down? My intention is to not worsen the ear infection and not disrespect.

If the pain is not withstand able, then, yes.
However, if you can raise the prostration place, such as putting a small table, and prostrating on it, then, you should do it.
